New York State Gold STAMP Pressure Ulcer Improvement Initiative SAVE THE DATE - Monday, October 4, 2010 10:30am to Noon
Leaders from the hospital, nursing home, and home care programs are spearheading a new, coordinated pressure ulcer quality improvement initiative called Gold Success Through Assessment, Management, and Prevention (STAMP) Program.
Pressure ulcers (PU) are one of the five most common problems experienced by patients in health care facilities and their incidence increased by 80% between 1993 and 2006*.
Pressure ulcers are both high cost and high volume adverse events, and most can be prevented by effective, coordinated care within and between settings of care.
The federal reform law, the Patient Protection and Affordable Care Act (PPACA), links payment and quality. Health care leaders must seize opportunities to move toward clinical integration through voluntary participation in care collaboratives that promote sustainable quality outcomes for patients throughout the continuum of care.
